Geriatric Care Consultants is a Los Angeles based private geriatric care management firm, designed to guide seniors and their family members through all aspects of the aging process. Our services range from single assessments to long term care plans and ongoing Care Management. Eldercare services are varied and are based on individual needs and tailored to your particular situation.

Assessments

This is 1-2 hour in-home meeting to determine a variety of issues – including:
• Independence level
• Home safety
• Cognitive status
• Signs of memory loss
• Medical needs
• Financial issues

Comprehensive Care Plan formation based on individual goals and needs (The whole process is 5-6 hours)
• Care plan adjustments

Monitoring
• Ongoing monitoring as status changes

Crisis Intervention
• 24 hour availability

Patient Advocacy and Education

Hiring and Screening Caregivers
• Ongoing management of caregivers schedules
• Oversight of caregiver quality and compatibility

Placement Services
• Recommendation and selection of appropriate facilities if needed

Counseling and Support

• Liaison between family members and local professionals
